Brief Description of WorkAs chair of Camden CCG I have been leading work on commissioning for ‘value’ (Porter and Lee) defined by patients’ needs, across whole pathways of care, and including non-health needs. Our Population Health management tool allowed segmentation according to patients’ health needs and delivery of corresponding patient-defined commissioning priorities through joined-up working between health and social care. Evidence from a number of areas – long term conditions, children’s services, frail elderly – demonstrated improved patient outcomes and reduced overall costs. System value is inextricably linked with values-based practice and future commissioning arrangements need to reflect and incentivize this.
